Growing upin Edmond, Stringer graduated fromEdmond Memorial High School, movingon to Oklahoma State University. There,she earned a bachelor of arts degree ineducation. For more informationabout OCCC, visit www.occc.edu./KLAHOMA S .URSING 4IMESLargest-Ever Trial Shows DirectOral Anticoagulants Are ViableOption for Treating Blood Clotsin Patients with CancerPeople with cancer face an increasedrisk for venous thromboembolism(VTE), blood clots that occur in deepveins like the legs or that travelthrough the blood stream and getlodged in the small blood vessels inthe lungs. Under current guidelines,cancer patients who develop VTEare prescribed low-molecular-weightheparin, an anticoagulant that mustbe injected under the skin daily forseveral months. While effective, thisregimen can be expensive and oftenburdensome for patients, leading manyto prematurely discontinue treatment.Results from the first largerandomized controlled trial comparingthis standard blood-thinning treatmentwith newer anticoagulants, DOACs,suggest the DOAC known as edoxaban— taken as a daily pill — work as wellas low-molecular-weight heparin andcould, therefore, offer an alternative andpotentially more palatable treatmentstrategy, according to researchers.Roughly one out of five peoplewith cancer develop VTE, whichcan cause pain and swelling whenclots occur in deep veins (known asdeep vein thrombosis) and breathingproblems, chest pain and even death ifthey move into the lungs (known aspulmonary embolism,). Anticoagulantsprevent the growth of existing clotsand prevent others from forming,but these drugs also elevate the riskof bleeding. Doctors must thereforebalance the risks of recurrent clotsagainst the risks of bleeding whentreating VTE.This study enrolled 1,050 individualswith cancer being treated for VTE at114 centers in 13 countries. Patientsrepresented a wide range of cancertypes and chemotherapy regimens;about 10 percent had blood cancersand the rest had solid tumors. Halfwere randomly assigned to receive lowmolecular-weight heparin (dalteparin)and half were assigned to receiveedoxaban. This treatment continued forup to 12 months. All patients werefollowed up for 12 months or untilstudy closure (minimum 9 months).The study was designed to assesstwo key factors. First, it evaluatedwhether edoxaban is at least as goodas dalteparin with respect to rates ofrecurrent clots and bleeding, the twomain risks associated with treatingVTE. Second, it was the first studyto measure the benefits of continuingtreatment with either drug for morethan six months after the initial VTE.The results confirm that edoxabanis not inferior to dalteparin withrespect to composite rates of recurrentclots and bleeding, the trial’s primaryendpoint, which occurred in 12.8percent of those receiving edoxabanand 13.5 percent of those receivingdalteparin. Although edoxaban wasassociated with a slightly higher rateof major bleeding, this was balancedby a slightly lower rate of recurrentVTE. Analyses of secondary outcomesand subgroups revealed that the twodrugs were identical in the rates of themost severe category of major bleeding,and that bleeding was most common inthe upper gastrointestinal tract and inpatients with gastrointestinal cancers.“For the vast majority of patientswith cancer-associated VTE, treatmentwith oral edoxaban can replace theinjectable dalteparin,” said lead studyauthor Gary E. Raskob, PhD, dean andregents professor of the University ofOklahoma College of Public Health.“Preventing VTE recurrence and majorbleeding can allow the oncologist toreally focus on the patient’s cancertreatment.”Raskob added that the resultsdo not necessarily apply to allDOACs because some act throughdifferent mechanisms or are metabolizeddifferently than edoxaban. Furtherstudies could elucidate which DOACsmight work best in the contextof different chemotherapy regimensand also illuminate optimal treatmentregimens for those with gastrointestinalcancers, he said.This study was supported by DaiichiSankyo. /KLAHOMA S .URSING 4IMES"%!#/.#ONTINUED FROM 0AGE a SANE nurse for a long time –she told me what it was all about,how much the program did forpeople,” Stringer said. “I also had afriend who worked for the Y(WCA)and a couple of friends who areadvocates.”That SANE nurse was actuallya sexual assault nurse examiner,a volunteer with Oklahoma CityYWCA and part of a program– in conjunction with DVNE, ordomestic violence nurse examiners that provide care to sexual assaultvictims and women involved in,and trying to escape from, domesticviolence situations.SANE and DVNE volunteers notonly care for patients’ physicalinjuries – they are there to providea sounding board for the emotionaltrauma they’ve experienced. Whileconducting a physical examination,nurse examiners collect forensicevidence for possible criminal actionagainst the patient’s attacker and,most importantly, they listen, Stringersaid.“Each case is different – this islife-changing for this person, andyou are there to make sure not*ANUARY only their physical needs are met,but also so they know they arenot alone,” she said. “I knew assoon as I took the classes, this wassomething I was meant to do.”Volunteers like Stringer arerequired to attend training andthen take eight-hour on-call shifts.Sexual assault victims wait for nurseexaminers in a private area and anadvocate is also on hand at the timeof the examination.“We make a difference becausewe are there – obviously, we arethere to take care of the physicalexamination, to make sure thevictims receive the best possiblemedical treatment, but there is somuch more to it,” said AmandaKemp, YWCA OKC director offorensic examinations. “There are theforensic aspects, the importance topreserve evidence and the emotionalcare for victims who can feelvictimized again if an exam isn’tconducted properly.”Nurse examiners can face anemotional tightrope in dealing withvictims. Some want justice, whileothers do not want to face everythingentailed in filing a police reportrelating to sexual and violent assault.Most importantly, they many timesneed support in a climate that might– perhaps, even subtly – blamethe victim for the incident; evenwith family and friends, there areoften feelings of shame that canlead those victimized down an evendarker path, Kemp said.“So many come forward andnobody believes them – how good aforensic nurse would I be if I didn’tbelieve them and didn’t collectas much evidence and informationas possible, whether they want toreport or they’re a case of nonreport,” she said. “There are also somany who blame themselves becausemaybe they were drinking, perhapsthere was a consensual situationthat led somewhere else.“Sexual assault is never a naturalconsequence – no one deserves tobe sexually assaulted,” the forensicexam director said.Stringer and her fellow nurseexaminers have a unique ability –not only to take on a second positionas a volunteer, but also one that canbe both physically and emotionallydraining at times. And, those nursesare needed – Kemp said YWCA’sSANE and DVNEs conduct about 425to 475 examinations a year.While that number is staggering,what is more alarming are thenumbers of women who never0AGE receive any kind of examination ortreatment, women who are afraidor embarrassed and never report anincident at all.“I think we should be doingthousands of exams,” Kemp said.“About 90 percent of college studentsassaulted don’t report for a varietyof reasons, and that’s frustrating andfrightening.”YWCA officials hope nurseexaminers like Stringer will turn thattide. Both sexual assault and domesticviolence nurse examiners work toempower victims, letting them knowthat no matter the circumstances oftheir assault, they are not to blame –and they are not alone.“Sexual assault is a communityissue that moves far beyond victims,their families and the programs thatare trying to help them,” Kempsaid.For Stringer, giving up the timeto work as a SANE is as much a giftto her as a service to those she ishelping, she said.“It is an incredible experience,it’s something that you can’t reallydescribe until you’ve been a partof it,” Stringer said. “Each shift issomething new, it is something thatmeans so much to each person – andit’s an honor to be a part of it.”
