CONTROLLED COPYTITLEHSE PERFORMANCE MONITORING AND MEASUREMENTDEPARTMENTREFERENCE NO.6.0QUALITY, HEALTH,SAFETY &ENVIRONMENTCo-X/QHS/SOP13DATE1st NOVEMBER 2017REVISION NO.2.0PAGE NO.Page 4 of 6PROCEDURE6.1Proactive and Reactive Monitoring6.1.16.2Co-X has incorporated both proactive and reactive monitoring of HSEmanagement system for performances in PICC.6.1.1.1Proactive MonitoringProactive monitoring is used to check compliance with theHSE program, operational control and applicable legislationand regulatory requirements, e.g. medical surveillance,chemical exposure.6.1.1.2Reactive MonitoringReactive monitoring is used to investigate, analyze andrecord environmental and OSH failures – includingaccidents, near misses, ill health.Measurement and Monitoring6.2.1To measure and monitor HSE performances, Co-X has established thefollowing (where required):6.2.1.1Environment / OSH objectives, targets and program.6.2.1.2Discharge to the effluent.6.2.1.3Reusable / non-reusable waste (if any).6.2.1.4Environmental / OSH Non-conformance.6.2.1.5Medical Surveillance.6.2.1.6Incident (accident and near miss).6.2.1.7Safety / Workplace Inspection.6.2.1.8Training and awareness.6.2.1.9Legal compliance, etc.6.2.2QHSE / Relevant HOD may work with the external parties (supplier /sub-contractor) to conduct the monitoring and measurement whererequired. Refer to the Appendix A – HSE Monitoring andMeasurement Matrix.6.2.3QHSE / Relevant HOD shall review and evaluate the reports receivedor given by the external laboratory / supplier / sub-contractor and makenecessary arrangement to initiate corrective or preventive action andimprovement program where area did not comply with theenvironmental / OSH regulations / legislation or requirements.6.2.4After review and evaluation of the reports, the reports shall be filed forreference purpose according to the retention period identified.

Measuring equipment that does not meetcalibrated shall not be used.6.3.3All certificates for the calibration of measuring equipment shall be filedfor reference purpose.6.3.4QHSE HOD shall ensure all the measuring equipment use to assessenvironment or safety condition, whether owned by Co-X / ExternalLaboratory / Supplier / Sub-contractor / Clients are timely calibratedbefore usage.Nonconformance and Corrective Action6.4.1In the event of non-conformity or potential non-conformity is detected,QHSE HOD shall initiate NCAR and submit to the respective HOD forfurther action.6.4.2The corrective and preventive action and evaluation results shall becompiled and if required under the legal and regulatory requirement tosubmit a copy of measurement results to the relevant authority onregular basis.RECORDS7.18.0QUALITY, HEALTH,SAFETY &ENVIRONMENTCo-X/QHS/SOP13N/AAPPENDIX / ATTACHMENT8.1HSE Monitoring and Measurement Matrix[Appendix A]
