This benefit is not applicable tothose individuals that were unemployed when they joined the association.Waiting PeriodThe Benefits Protector Program is available to Members that have been aMember of USA in good standing for at least six (6) months. Please refer to theMembership Agreement for additional information on Good Standing.Benefit ActivationeciProof of LossmenThe member must notify USA within 20 days of the loss of employment. Noticemust be submitted to USA at the following address:2221 East Lamar, Suite 900Arlington, TX 76006Upon receipt of the written request, USA will send the member a BenefitsProtector application to complete and return to USA .SpOnly those applications where USA is able to validate and confirm the reasonwhy the Member lost employment will be accepted. If loss of employment wasthe result of misconduct connected with the Member’s work or if the memberquit his/her job voluntarily, the member is not entitled to receive this benefit.Effective DateThe effective date of the Benefits Protector will be equal to the last paid to date ofthe Member’s USA membership prior to the loss of employment.Termination DateThe termination date of the Benefits Protector is 90 calendars days from theeffective date. To continue benefits beyond this date, the member must submitthe required membership dues. The organization was formed topromote the adoption of equitable health care policy in the United States,engage in nonpartisan research, study and analysis for the benefit of thegeneral public regarding the health care system and on occasion; publish theresults of our research.enUSA is committed to the promotion of equal access to health care for allAmericans. In order to ensure equal access to health care, it is important thatthere are sufficient medical providers available to serve the public. For thatreason, USA developed a scholarship program. This program will providescholarships to outstanding high school seniors that show promise of continuedacademic performance. This program is only available to dependent childrenand grandchildren of USA members and will allow deserving students toattain their educational goals.AwardSpecimUSA will award five (5) scholarships each academic school year.The scholarships are payable at 1,250 per semester and 2,500per school year, for a total of four semesters (Fall and Spring) anda total award of 5,000. The award is only applicable to studentswhose degree program is in a medical related field.EligibilityMembers that have maintained good standing for at least sixconsecutive months are eligible to submit a scholarship applicationon behalf of their dependent children and grandchildren.Good StandingGood Standing shall mean the status of a Member whose Membershipdues have been paid current and no membership dues are or havebeen past due. If the Member neglects to pay the required dues inany given month, the Member is not eligible.Promotion/PublicityAward recipients will be listed in the USA Total Health andWellness Magazine. The selection criteria that will be used willinclude the following: Academic achievement, including grades, rank in class,standardized test scores and achievement test scores.Student must have a minimum GPA of 3.0 Community/extracurricular involvement Leadership Participation in specific activities Awards and recognition Work historyApplication ProcessApplications are available year round and can be obtained bycontacting USA at 800-872-1187. No later than June 30th,applicants must complete and submit an application along with thefollowing documents: ACT or SAT scores High school diploma Letter of acceptance from the educational programdirector High school transcriptScholarship Notification ProcessThe scholarship review committee will review the applications.Final determination regarding the award will be made by July 23rdand notification letters will be sent via US mail. Award checks willbe made payable to the college/university.Scholarship Renewal Award CriteriaAfter the initial award is extended for the first semester, in order tocontinue to receive an award for the following subsequent semester,the student must provide a certified copy of their transcript thatreflects a minimum GPA of 3.0 with at least 12 hours earned. Thisprocess must be completed for the three semesters that follow theinitial application and award process.21
