Once submitted, your electronic or wet signature will be attached at the bottom of page 5.You will be held accountable for all of the information below, so it is in your best interest to read everything here before submitting the contract.Your signature, whether electronic or wet, verifies that you have read and understood all policies and procedures related to having a meal plan.Pages 1 - 5 must be submitted to the Meal Plan Office and constitute the meal plan contract. Please retain the paymentschedule and a copy of this contract for your records.1. Contract Term: By signing this contract, you are agreeing to participate in the meal plan program for the Fall 2021 andSpring 2022 semesters, and are financially responsible for all meal plan fees. Students applying in Spring 2022 will befinancially responsible ONLY for the Spring 2022 semester.2. Your application fee of 50.00 is due with the first monthly installment. Financial Aid cannot be used to make thispayment. This fee is a 50 non-refundable processing fee. If you are applying through the Student Housing Portal, thisfee is due at time of application.3. Cancelling your meal plan: The deadline to cancel the Meal Plan is Friday, September 3, 2021. Should you need tocancel your meal plan for any reason, you will need to complete a request to cancel meal plan form. These can be foundonline at the Dine on Campus website: https://dineoncampus.com/csun. Students living in a non-kitchen unit are noteligible to cancel their plan. All cancellations must be submitted in writing via a cancellation form. Verbal cancellationsare not valid. Once the Fall or Spring (for those applying in Spring) semester begins, a 60.00 cancellation fee will beapplied to all meal plan cancellations regardless of reason for cancellation.a.Cancelling before the semester begins: Should you decide not to attend CSUN you will need to submit acancellation request to ensure you do not get charged for a Meal Plan. Once we have received a completedcancellation request, we can begin the 4-6 week process to cancel your plan. If a refund is due, you will receive it atthe end of the 4-6 week process, via e-Refund, paper check, or direct refund to the credit/debit card used to pay therefunded fee(s).Cancelling after the September 3, 2021 deadline (for Fall 2021) or Canceling after the semester begins (forSpring 2022 applicants): Once the deadline has passed (Fall applicants) or semester has started (Spring applicants),you may only cancel your meal plan if you are no longer attending CSUN, have cancelled your housing contract tolive off-campus, or have a medical reason supported by a Doctor’s note. We must be able to be verify that you havewithdrawn from CSUN or have moved off campus in order to cancel your meal plan. Approved requests will becharged a 60.00 cancellation fee and an amount for each week spent on the meal plan regardless of card usage.Cancellations based on financial hardship, not using the plan, having a kitchen and deciding to cook ormoving from a non-kitchen unit will not be approved.Not using your meal plan card is not the same as cancellation. You are still responsible for the meal plan feeswhether your use your card or not.b.c.d.4. Changing your meal plan: The deadline to change your meal plan in the Fall semester is Friday, September 10, 2021and Friday, January 7, 2022 in the Spring semester. Spring applicants who apply after the deadline will have the firstweek of school to make a change if needed.a.Students are permitted one change per semesterSpring changes may be submitted starting in December, changes before then will not be approvedb. Changes submitted after these deadlines will not be approvedi.Some exceptions apply, please speak to the Meal Plan office for more informationc. Students living in non-kitchen units are not eligible to change their plan to a Matador Blocki.If the student moves from a non-kitchen to a kitchen unit, then they may change their meal plan option to a blockplan. The student will have one week after moving to make the change, changes received after the one-weekperiod will not be approved.i.5. Payments for Fall Applicants: Will be posted in 8 installments to your myNorthridge portal. 4 installments for the Fallwill be due September 1, October 1, November 1, and December 1. 4 installments for the Spring will be due February 1,March 1, April 1, and May 1. Payments for returned checks and fines must be paid by cash or certified funds only.6. If you have Financial Aid: Keep in mind that the University applies aid FIRST to tuition & fees, SECOND to housing,and LAST to meal plan. If your financial aid is not posted by the payment due dates or is not enough to cover all mealplan costs, you will be responsible for making payments as they come due. Any adjustments or changes in Financial Aidwhether imposed or voluntary will not nullify any financial obligations. If you receive a Financial Aid disbursement priorto meal plan charges being posted or paid, the student must utilize these or personal funds to pay for any outstanding fees.Full and prompt compliance with all Financial Aid office requests for information and response is STRONGLY advisedto ensure a timely financial aid award and distribution.7. Late Fees: Payments are due on the 1st of the month according to the payment schedule, if your payment has not postedby the 10th of the month, your portal will be charged a 15 late fee for every payment that is missed AND may result inmeal plan suspension. If your meal plan privileges become suspended, you will need to bring your meal plan accountbalance current by making a payment either online through your myNorthridge Portal or in person at University CashServices. Once payment has been confirmed by the Meal Plan office your meal plan privileges will be reinstated. You arestill responsible for payments while your card is deactivated, and any missed meals will not be reimbursed.a.Failure to make timely payments will result in:i. Assessment of late fee(s) as stated on payment schedule.ii. Suspension of meals without compensation for missed meals.iii. Withholding of University services pursuant to Section 42380, et seq., of Title 5 of the California Code ofRegulations. This includes: 1) withholding of grades or diploma and 2) denial of registration .8. Email is the Meal Plan Office’s primary method of communication. Please be sure to provide us with the emailaddress you check most frequently. Occasionally we will call you, if we find your lost card or you leave your wallet orother valuables at a Matador Eats location; so please be sure to provide your cell phone number on the contract. Our emailaddress is mealplan@csun.edu9. You will be provided with a Meal Plan card that is loaded with your meal plan choice. This card will contain 3 mealcomponents: Meal Swipes to be used at G’mos & Bamboo Terrace, Exchange Swipes to be used at any dining location oncampus (restrictions apply) and MataCash to be used at all other dining and retail locations on campus.Meal Swipes: Meals to be used at G’mos & Bamboo Terrace only.i. All-Access Plan: Meal Swipes are unlimited. Students can enter G’mos & Bamboo Terrace, as many times asthey would like; however, swipes are non-transferrable.ii. 10 & 12 Meal Plan: Meal Swipes have a weekly limit and are non-transferrable. These meals do not rolloverfrom week to week; they are use or lose.iii. 200 Block Plan: Half of total meal swipes are allotted at beginning of each semester; they do roll over fromweek to week and can be used to bring in guests.iv. Meal Swipes CANNOT be converted to MataCashb. Exchange Swipes: Meals to be used at any dining location on campus (restrictions apply).i. All Meal Plans come with Exchange Swipes: They each have a weekly limit and are non-transferrable. Thesemeals do not rollover from week to week; they are use or lose.ii. 200 Block Plan: comes with 10 exchange swipes per semester; they do roll over from week to week.iii. No daily or time limit. Menus available per location, restrictions apply.iv. Meal Swipes CANNOT be converted to MataCasha.c.MataCash: Is cash value (dollar for dollar) and can be used for on-campus meals, snacks, drinks, or any purchase atany of the dining and retail locations on Campus.i. Allotted MataCash: The number of MataCash in the name of your plan is the amount for the entire academicyear. This amount will be split in half and loaded to your account at the beginning of each semester. If youdivide the total loaded to your account at the beginning of each semester by 16 (these are the total weeks per3

This tells you theamount of MataCash allotted per month (portion per monthly payments).ii. Rollover: Matacash rolls over from semester to semester, but must be used by the end of the academic year(May). Unused Matacash will be forfeited and will not be transferred or refunded.iii. Additional MataCash: You may purchase additional MataCash any time online through the Matador EatsAccount Center using a debit or credit card. These will appear in a separate account on your card, which willnot be used until all allotted MataCash is spent.iv. MataCash CANNOT be converted into Meal SwipesGuest Passes: Students on the 10, 12, or All-Access plan will receive 3 Guest Passes per semester to be used forfamily or friends at G’mos & Bamboo Terrace. Each guest pass will allow 1 guest to dine with you, and are valid forthe entire 2021-2022 academic year. You must have an active meal plan to utilize the free guest passes.10. Due to limited hours of operation during winter and spring Breaks, meal swipes are not available during thesetimes.a.b.Matacash will be active during these periodsThe cost of your meal plan does not include meal swipes during Winter and Spring breaks11. If you select or are placed into a housing unit without a kitchen, you are required to purchase at minimum the 10or 12 Meal Plan.a.If you do not want or cannot afford a meal plan: you should not accept assignment to a non-kitchen housing unit,as there are NO exceptions to this requirement. Please contact the Housing Office to see about changing your roomassignment BEFORE the semester starts.12. Students living in kitchen units are welcome to purchase a meal plan. However, they cannot later cancel the mealplan unless they move out of on-campus housing to an off-campus location or are no longer attending CSUN.13. The Matador Block Plans are only available to students living off-campus or students living in a kitchen unit inStudent Housing.14. Food Allergies/Dietary Restrictions: We will do our best to accommodate special or restrictive diets; however, there isno guarantee that we will be able to satisfy all dietary requirements. There will not be a reduction of food service fees fordietary or other related problems.a.Please contact us to further evaluate your specific requirements BEFORE applying for a meal plani. If you have strict dietary requirements, which require you to purchase a special food and/or prepare it yourself,you should refrain from purchasing a meal plan and/or accepting a non-kitchen housing unit.15. If you lose your meal plan card: Contact the Meal Plan Office right away so your card can be deactivated to preventunauthorized use.a.b.c.d.e.Lost Card replacements cost 10.00; this is due at time of replacement.i. This fee can be paid with your MataCash, Exact Cash, or a Credit/Debit cardDuring Regular Business Hours: You will need to complete the online form, which can be found at the Dine onCampus website under Meal Plan Forms: https://dineoncampus.com/csun. The meal plan office will email you onceyour card is ready for pick up.During Regular Business Hours: Stop by the Meal Plan Office to pick up your new card with a new number afteryou receive confirmation that your card is ready for pick up.After Regular Business Hours/Weekends: Stop by G’mos/Bamboo Terrace for a temporary card that will last threedays. They will ask for the confirmation email to make sure you’ve requested a new card, you will receive aconfirmation after filling out the Lost Card Meal Plan Form online. The meal plan office will contact you by emailwhen your card is ready for pick up during their regular operation hours.i. Please bring the temporary card AND form of payment if not using MataCashThe Meal Plan office is not responsible for unauthorized use if you do not notify us that your card has beenlost or stolen.16. All decisions regarding the Meal Plan Program shall be made by the Meal Plan Office.
