Servicescan be authorized for children and youth adults under 21 years of age who haveeligible medical conditions (such as congenital heart disease, sickle cell anemia,cystic fibrosis, cleft palate, and HIV infection) and whose families are unable to payfor all or part of their care. CCS determines eligibility and selects the most qualifiedprofessionals to treat each child‘s CCS-eligible condition.A child may be eligible for CCS if she or he meets all of the four following criteria:1. Age - The child must be under 21 years of age.2. Residence - The child must be a permanent California resident.3. Family Income – The child‘s family income must meet one of the followingcriteria. Be less than 40,000 per year (adjusted gross income as reported onCalifornia State income taxes) – OR If greater than 40,000 a child may be eligible id medical care for theCCS-eligible condition is expected to cost more than 20% of thefamily‘s income. EXCEPTION: Income is not a factor in determining eligibility for the- 11 -

medical therapy program. Eligibility for the medical therapy programis based on medical condition and clinical findings.4. Medical Conditions – Only certain conditions are eligible for CCS.
