Transitional Care Journal of Christian Nursing118 April/June 2016, Volume 33 Number 2 , p 112 –ABSTRACT:The Medicare mandatory readmission reduction program hashospitals scrambling to reduce 30-day readmissions. AFaith Community Nurse (FCN) Transitional Care Model wasdeveloped from systematic literature review of predictivefactors of readmission and pre- and post dischargeinterventions that decrease readmission. The model presentsspecific FCN care that occurs pre- and post-hospital dischargeto support the patient in transitioning from one level of careto another, move toward wholistic health, and avoidunnecessary readmission.

Transitional Care Continued Article ContentHospitals have been scrambling in recent years to reducereadmission rates. With passage of the Affordable Care Act in 2010,Medicare was required to set up a readmission reduction programto lower readmission rates after hospital discharge.Payment penalties began in October 2012 for hospitals subject tothe Inpatient Prospective Payment System (IPPS). Hospitals lost upto 1% of every Medicare payment if the hospital had an excessive30-day readmission for only three specific diagnoses-acutemyocardial infarction, congestive heart failure, and pneumonia.The penalty increased to up to 2% for 2013 readmissions and 3% for2014 readmissions. In 2015 exacerbation of chronic obstructivepulmonary disease (COPD) and total hip and knee arthroplasty wereadded as additional diagnoses to measure hospital performance. In2017, coronary artery bypass surgery will be added (Centers forMedicare & Medicaid Services, 2015). Modern Healthcare reportsthat only 799 of more than 3,400 IPPS hospitals performed wellenough in 2015 to avoid penalties in 2016 (Rice, 2015).
