LIT REV-UNSAIDs (Including Aspirin): PrimaryPrevention of Gastroduodenal ToxicityBy Chandra Murali, Illinois Central College RN Student,Graduated May 2019Ulcers are one of the manycommon diseases seen inadult patients. With the useof NSAIDs for cardiovascularprotection and antiinflammatory effects, themorbidity and mortalityassociated with gastric and duodenal mucosal injuriesare rising. Authors Drs. Feldman and Das explain inthis article the primary prevention of gastroduodenaltoxicity for patients using NSAIDs. Some preventionapproaches include using (a) NSAID together with a PPIor misoprostol or (b) using coxib with or without a PPI. Ahigh-dose H2 blocker can be used, but it is reserved forpatients who are not able to tolerate PPIs or misoprostol.Patients taking NSAIDs long-term should be examinedand treated if there is unexplained blood loss anemia,iron deficiency, significantdyspepsia or GI bleeding. Furthermore,patients with a history of peptic ulcers shouldbe tested for H.pylori before starting long-termuse of NSAIDs. As highlighted in this article, it isimportant to analyze risk factors in patients beforeprescribing NSAIDs for long-term use. Risk factorsinclude: history of ulcer disease or ulcer complication,use of dual antiplatelet therapy, use of anticoagulanttherapy, age 60 years, glucocorticoid use anddyspepsia or gastroesophageal reflux disease symptoms.Feldman, MD, M. and Das, MD, S. Led by the Nursing Student ProgramManager, Laura Dailey, the science behind mindfulness is reviewed andKindred clinical leaders discuss how mindfulness supports their work withpatients in the hospital.
