PROGRAM ARTICULATIONAGREEMENTBetween theDENTAL HYGIENE PROGRAMHARPER COLLEGEPALATINE,)LLINOISAnd theDENTAL HYGIENE PROGRAMCOLLEGE OF APPLIED SCIENCES AND ARTSSOUTHERN ILLINOIS UNIVERSITY CARBONDALECARBONDALE, ILLINOISIn an effort to provide a continued, articulated higher education baccalaureate degree program that will build oncommunity college and university learning experiences, and also eliminate duplication of instruction, this agreementis set forth between the Dental Hygiene program at Harper College and the Dental Hygiene program in the Collegeof Applied Sciences and Arts at SIUC with the approval of the Board of Trustees of Southern Illinois University onbehalf of Southern Illinois University Carbondale (SIUC).All graduates of Harper College with an A.A.S. degree in Dental Hygiene who meet SIUC entrancerequirements will receive preferential status when being reviewed for the controlled enrollment process intoSIUC's Bachelor of Science (B.S.) degree in the Dental Hygiene program in the College of AppliedSciences and Arts, SIUC. A separate Dental hygiene application is required in addition to SIUC's generaladmission application. Any student transferring from outside the University or from other SIUC programsinto the Dental Hygiene major will be evaluated on the Dental Hygiene application information. Thispreferential status will be based on space availability. Students accepted into the Dental Hygiene programmust meet the program's controlled enrollment criteria.2.*A Harper College graduate receiving an A.A.S. degree in Dental Hygiene at Harper College and who isadmitted into the Dental Hygiene program at Sill C will be accepted for the Capstone Option if thefollowing are met:(a)(b)(c)(d)a minimum of 60 transferable hours to sruC.an overall grade point average (GPA) of2.25 or above (4.0 scale) on allaccredited work received prior to the awarding of the AAS degree ascalculated by sruc's grading regulations.the completion of at least ( 15) hours of transferable University CoreCurriculum (general education) credit at Harper College.all Dental Hygiene major courses are completed with a grade ofC orhigherAcceptance into the Capstone program guaranteesthe A.A.S. degree recipient in the DentalHygiene program at Harper College completion of the B.S. in Dental Hygiene degree with a totalof no more than 60 semester hours of work at SIUC. Harper College students must complete theSIUC Capstone Application in addition to the SIUC undergraduate admissions application, theDental Hygiene selective admission Application, and any other selective admission criteria to beadmitted under this Option.Harper College graduates receiving an A.A.S. degree with a minimum of 60 transferable hours to SIUC inDental Hygiene with less than a 2.25 overall GPA, but with an overall GPA of2.0 to 2.24 (on allaccredited work using SIUC's grading regulations) will be eligible for admission based on spaceavailability to the Dental Hygiene program at SIUC. These students may require more than 60 semesterhours to complete their Dental Hygiene Bachelor of Science degree.

Dental Hygiene at Harper College and Dental Hygiene at SIUC Articulation AgreementPage 24.Harper College students transferring to the Dental Hygiene baccalaureate degree program at SIUC whohave not completed all of their A.A.S. degree requirements at Harper College will have their related coursework evaluated on a course-by-course basis by the appropriate ASA program.5All students will be required to complete 60 senior institution hours, the last 30 hours at smc, and at least120 hours with an overall GPA of2.0 to receive a Bachelor of Science degree in Dental Hygiene. Coursework may include University Core Curriculum as vfell as Dental Hygiene major courses.6.An SIUC Dental Hygiene representative will communicate periodically with Harper College personnel inDental Hygiene for general advisement and degree planning purposes.7.Upon successful completion of all degree requirements, and following all policies and regulations stated inthe program and university guidelines, Harper College students will be eligible to receive the Bachelor ofScience degree in Dental Hygiene, College of Applied Sciences and Arts, Southern Illinois UniversityCarbondale.8,Should changes occur in course or program content, the school making the change agrees to notify theother school in writing so that this agreement can be re-evaluated. Notice of changes shall be given atleast 45 days prior to the beginning of the semester when the change is implemented.November 1,2007
