FOREWORDPharmacotherapy has always been an integral part of healthcare system andwill remain so in the future.With the advent of innovative interventions itsrole has become even more critical and requires the continuous availabilityof quality, effective and affordable pharmaceuticals. Pharmaceutical Fundand Supply Agency is the public body entrusted with this role in Ethiopia.Though the Agency has been working hard to ensure the continuousavailability of needed pharmaceuticals at service delivery points, thereis still huge gap between demand and supply in pharmaceuticals. So far,there is no defined and agreed upon list of pharmaceuticals that theAgency procures and avails to public health facilities. As part of itstransformation, the Agency has been working on preparing a defined listof pharmaceuticals that will guide its procurement and supply. After seriesof consultations with stakeholders involving relevant experts in each areafrom health facilities, FMOH, RHBs, EPHI, EHIA, development partnersand the Agency, first edition of the list is now ready for use.We are proud to introduce to you this first edition of the PharmaceuticalsProcurement List which has got buy-in by all stakeholders. Properimplementation of the list is expected to bring about significantimprovement in the availability of pharmaceuticals at service deliverypoints.The list will set a reference against which the Agency's performancecan be measured, and health facilities and other stakeholders know whatcan be availed through PFSA. It will also encourage the private sector'sinvolvement in availing pharmaceuticals which are not included in thislist and also persuades them to healthy competition. The list containsall categories of pharmaceuticals with full and standardized descriptionof each product. Users of the list are expected to consistently use thedescriptions used in the list in all communications and transactions theyundertake for smooth exchange of information along the supply chain.The list will be revised every two years in line with advancements in themedical and pharmaceutical sciences, in consultation with stakeholders.II

PHARMACEUTICALS PROCUREMENT LISTINTRODUCTIONThe continuous availability of pharmaceuticals is key for the provision ofquality health services. Pharmaceuticals Fund and Supply Agency (PFSA) isa public institution established to ensure continuous availability of qualitypharmaceuticals at an affordable price to public health facilities through needbased pooled procurement. The Agency has made remarkable achievementsover the past years in terms of procurement, distribution, inventorymanagement and warehousing. The type and volume of pharmaceuticalsthe Agency procures and distributes has exponentially increased over thepast years. The value of annually procured pharmaceuticals has increasedby more than 80% in five years’ time (2003 to 2008 E.C.). The value ofpharmaceuticals distributed by the Agency in 2009 E.C. has increased byabout 140% as compared to that of 2003 E.C. In addition to the productsit procures, the Agency also handles the storage and distribution of hugeamount of program products donated by development partners.With respect to infrastructure, a total of 17 modern warehouses wereconstructed at the center and branches of the Agency during the last fiveyears. This has increased the Agency's warehousing capacity to 305,910 m3from the 16,760 m3 that was available when the Agency started operation.The inventory management across the supply chain has shown significantimprovement through implementation of the Integrated PharmaceuticalsLogistics System (IPLS) and the Health Commodity Management InformationSystem (HCMIS). The Agency, in collaboration with stakeholders anddevelopment partners, has also been involved in capacitating the supply chainworkforce at all levels through in-service training, supportive supervision andmentoring.Tens of Thousands of professionals have taken trainings in varioussupply chain areas. With the objective of making itself more accessible tohealth facilities, the Agency has increased its branches to 19. The Agency hasintegrated the supply management of most of the program pharmaceuticalsand is expanding the directly delivery of such products to service deliverypoints. These efforts have resulted in better availability of both revolving andprogram pharmaceuticals in the country.1

Regardless of the efforts made and achievements gained so far, there is stilla huge gap in ensuring the continuous availability of needed pharmaceuticals,this signposts a lot to be done to continuously avail needed pharmaceuticals atservice delivery points. To this end, the Agency has been involved in BusinessProcess Reengineering (BPR) study which enabled the Agency to identify thekey performance gaps and design interventions to optimize its operations.The Agency has already started implementation of the BPR at head office levelthrough restructuring of some sub-processes. The BPR study has identifiedthe absence of an agreed upon procurement list as one of the causes for theinterrupted supply of pharmaceuticals. Though the Agency was establishedto avail essential pharmaceuticals, it has been trying to procure and avail alltypes of pharmaceuticals as per annual requests made from health facilities.This has put the Agency unfocused and having no reference list against whichits performance can be measured despite the increasing efforts being made.This necessitated the need to define the list of pharmaceuticals the Agencyprocures and supplies to public health facilities on regular basis.Defining the list will enable the Agency to be focused on procuring, storingand distributing pharmaceuticals that are essential for the provision ofhealth services for majority of the population.This will improve the Agency'sperformance and result in better availability of these selected pharmaceuticalsand reduce wastage. Health facilities will be clear on what can be supplied bythe Agency on regular basis and look for options to avail products that arenot supplied by the Agency.To this end, the Agency has been refining the list of pharmaceuticals usedfor the annual quantification and procurement on the basis of the typesof pharmaceuticals sent to the Agency by all public hospitals and selectedhealth centers as part of their annual requirement request. It is believed thatthe annual requests from these health facilities have gone through series ofconsultations within each health facility and got approval of the health facilitiesDrug and Therapeutics Committees (DTC). The list of pharmaceuticals(medicines, medical supplies, medical equipment, and laboratory chemicalsand reagents) included in each category of the draft list were criticallyreviewed by relevant experts in the consultative workshops organized.The draft medical equipment list was reviewed in a consultative workshop byexperts of all clinical categories and other relevant professionals from2

PHARMACEUTICALS PROCUREMENT LISTreferral, general and primary hospitals, health Centers, RHBs, FMOH andEPHI. Medical equipment are categorized into capital and non-capital groupsbased on their complexity. Capital equipment are those that require signingofficial agreement with the Agency to initiate the procurement process. TheAgency has decided that tenders for the procurement of capital equipmentwill be floated twice per year to minimize the burden associated with therepetitive tendering in practice so far. Consultative workshop was alsoorganized to review the draft list of laboratory chemicals and reagentsby laboratory and pharmacy professionals from hospitals, health centers,EPHI, RHBs and the Agency. While reviewing the list of products in eachpharmaceutical category, due emphasis was given for standardization ofdescription of each product, in addition to addition and deletion of productsinto/from the draft list.Based on the reviews made on the draft list, descriptions of each productin the Pharmaceutical Directory Service (PDS) of the Agency is revised andstandardized to be used as a reference standard in all operations of theAgency. Following this, final review workshop was organized to finalize thelist. Experts from referral hospitals, RHBs, the Agency and developmentpartners took part in the review workshop and made final polishing on eachcategory of pharmaceuticals.Finally, the list was coded with the national base code in a consultativeworkshop by involving relevant professionals (pharmacists, laboratorytechnologists and engineers) from selected APTS implementing hospitalsin the country. In the base code, the first four letters indicate the parentcompound of the product, the second two numbers indicate both thestrength/length/volume, dosage form and use category of the product. Thebase code is the same for a single product with one generic name whatever the batch, price or country of origin is different. However, for a singleproduct which has the same generic name, the price code, batch numbercode, expiry date code, country of origin code and receiving date code thatwill be entered automatically with the product varies. The code will be usedfor insuring transparency by facilitating auditing of products transacted fromsuppliers up to users (patients).3

The efforts made by the Agency with continuous support from stakeholdersand partners resulted in realization of this first edition of PharmaceuticalsProcurement List for the Agency that has got buy-in by all stakeholders. TheList will be officially communicated to FMOH, RHBs, and health facilitiesfor their reference. The Agency will exert maximum effort to ensure thecontinuous availability of the pharmaceuticals included in the list and thelist will be updated every two years in line with local and global changes indisease management. The Agency will also strengthen its collaboration withall supply chain players to fully realize the goal the list is developed.4
