Cast iron Main, 10 miles Ductile Iron Main 1 mile other main. 31,262steel services, 7,320 Copper services, 93,947 PE services, 65,257 other services.FINDINGS:At the time of this report it appears MUD violated 49CFR Part 192.605 by not following theirOperations and Maintenance procedure by not marking the location of their underground naturalgas pipeline and performing inspections of excavaiton activities to prevent damage to pipelinefacilities.CONCLUSION:At the time of this report it appears the buried pipeline was damaged at 1823 Corby Street inOmaha on May 10, 2013 due to Metroploitan Utilities District Procedures L-614.0 (I)(F)&(G)notbeing followed.BODY of Report: On May 10, 2013 I spoke with Tom SMITH on North 19th Street betweenCorby and Miami Streets. SMITH said locate order 131230933 had been called into the one callcenter. SMITH said he was excavating for a sewer at 1823 Corby street when he hit theMetropolitan Utilities District natural gas main. SMITH said there were no paint or flagsmarking the Metropolitan Utilities District (MUD) main that was struck by his excavationactivity. SMITH said when he hit the gas line he called his supervisor Mike FRIEND who calledin the damage report and received the damage report number 131300491. SMITH said the paintand flags I observed on the scene were put down at the time MUD engaged in their emergencylocate activities. SMITH said FRIEND was the supervisor for this project and had made all thecontacts with the one call center. SMITH had a copy of his one call locate order and a map thathad been sketched by a Janke Plumbing employee which did not show buried natural gasfacilities on the property.SFM Damage Prevention

NEBRASKA STATE FIRE MARSHAL - PIPELINE SAFETY SECTIONOn May 10, 2013 while I was observing the paint marking the natural gas line running under thesidewalk on the east side of 19th Street between Corby and Miami Streets I met Joe THOMAS.THOMAS said he had been out on the sidewalk early that morning and there were no paintmarks on the sidewalk. THOMAS said he had been evacuated from the area, was just returning,and this was the first he saw of the paint marking the gas main.On June 10, 2013 I reviewed MUD's copy of Locate order 131230933 which stated on May 6,2013 Ron JANKOWSKI painted and flagged the locate order at 1818 and 1823 Corby Street outfront, 37 feet both sides of the property at 1631 hours.On June 10, 2013 I reviewed the Field Services Form 8 leak investigation report completed byAlvin CLARK.
