When searching for providers, members must look for Encore PPO as a product under the provider’s name. Encircle EPO NetworkUnderstanding the importance of controlling medical costs,Encore has designed the Encircle EPO Network to addressthose costs. With 60 plus hospitals and over 21,000 provider locations, Encircle is designed specifically to deliver maximum savings for our payors, employers and members inexchange for a smaller quality-based network. Whensearching for providers, members must look for Encircle EPO as a product under the provider’sname. Encircle/Encore Combined NetworkThe Encircle/Encore Combined Network offers the best ofboth worlds: deep savings through Encircle and comprehensive network access through Encore. When searchingfor providers, members should use an Encircle providerfirst to get the deepest discount and highest benefit reimbursement from your healthcare payor. However, themember can use any provider in the Encore PPO Network, but may received reduced benefit reimbursement from their healthcare payor.A

For questionsabout your benefits, call your employer or the benefits/eligibility telephone number listed on your memberidentification card.WHEN YOU SEEK SERVICES, ASK:1. Is the provider part of the Encore Health Network PPO or Encircle EPO?2. Are the lab, x-ray or other ancillary services being performed by a facility that participates in Encore Health Network PPO or Encircle EPO?3. Does the anesthesiologist participate in Encore or Encircle?If your provider is not listed in this directory, ask if they are part of Encore Health NetworkPPO or Encircle EPO. If not, fill out a Physician Nomination Form at the back of this directory and fax it to us. Choosing a doctor also means choosing a hospital. To maximize savings and avoid inconvenience, always check to make sure your physician has admitting privileges at the hospital you wish to use forinpatient hospitalization. HOW TO GET CARE OUTSIDE THE ENCORE PROVIDER NETWORKThrough access to this network, your employer or insurance company offers a benefit plan, which provides higherlevel of benefits if you use health care providers participating in the Encore network. Additionally, Encore providers agree to accept certain fee arrangements that result in savings over their standard charge rates. This givesmembers the chance to save in two ways.If health care services are received from a health care professional or hospitals that are not participating in Encore, the level of reimbursement will be reduced.ASSESSING EMERGENCY CARE/URGENT CAREFor questions on emergency and urgent care, contact your insurance company or your employer’s benefit plan’soffice.
