DEAR LEADERSHIP FORUM PARTICIPANTSI am pleased to welcome you to Boston for our second Leadership Forum:Putting Your Money Where Your Mouth Is 2.0. On behalf of the HarvardSchool of Dental Medicine Initiative Integrating Oral Health and Medicine,we are thrilled to bring together some of the most influential leaders inacademia, healthcare, and business to address the bold idea of integratingdentistry and medicine for the greater benefit of all.According to the Journal of the American Dental Association, 1.3 million U.S. emergencyroom visits annually are due to nontraumatic dental conditions. The most vulnerablein our society are disproportionately affected by this disconnect between preventativedental and medical care. Studies have shown that low-income individuals, minorities,and the elderly are most at risk of not having access to dental care. This Forum willaddress why this is happening and propose innovative solutions.We will hear from Marleece Barber, MD, MSPH, Chief Medical Officer, Lockheed/Martin and Board Member, National Business Group on Health, who will deliverour keynote on “What Integrating Oral Health and Medicine Means to Business andEmployers.” Barber, and other industry experts, will look at the cost of inaction.Faculty from Harvard School of Dental Medicine, Harvard Medical School, andHarvard T.H. Chan School of Public Health will explore models and academic studiesthat make the case for integration, and present a vision for “An Integrated Practiceof the Future.” Policy experts and economists will discuss data that point towardintegration as important to the future of health care delivery.Over the next two days we can look forward to lively and informative speakers,thought-provoking discussion, and the opportunity to connect with like-mindedcolleagues. She also serves as theChief Medical Officer for the Lockheed MartinCorporation. She is responsible for designing,developing, and executing a health andproductivity strategy that includes wellness andpreventive care for over 100,000 employees.Prior to joining Lockheed Martin, she wasemployed by Deere & Company where shecreated wellness programs and initiatives thatemphasized the importance of risk factorreduction in the prevention of chronic disease.She was also the Director of Global Safety,ensuring that appropriate standards were metand programs deployed to prevent workplaceinjuries. He was the co-director ofthe Harvard University’s Dental GeriatricFellowship Program and a clinical instructorin their Department of Oral Health Policyand Epidemiology. Currently, he a member ofthe National Eldercare Council that advisesthe American Dental Association and otherorganizations on oral health policy and newmodels of care for the aging population inthe United States. Dr. Brennan attended TuftsUniversity School of Dental Medicine andcompleted a Harvard University GeriatricDental Research and Teaching FellowshipProgram. He was recognized for hispostdoctoral work at Harvard as the recipientof the Dr. James Dunning Award for excellencein biomedical research and teaching. Hecompleted a second post-doctoral fellowshipin Medicine and Palliative Care at Mt. SinaiHospital in New York and is a graduate ofthe American Society on Aging LeadershipInstitute in San Diego, California. Since 1982,he has taught oral health for the MedicalGeriatric Fellowship Program at Maine MedicalCenter. He has had the opportunity to teachat Harvard University Medical and DentalGeriatric Fellowship Programs, HarvardUniversity School of Dental Medicine, theUniversity of New England Medical School,Northern Haiti Dental Society, and at the 2013Maine Geriatric Conference. Locally, he hasbeen involved in many community dental andmedical organizations and as a volunteer inthe Dental Lifeline Network for which he wasnationally recognized by CNN Money magazinewith the 2014 “Hero Award” for his work withthe elderly and other fragile individuals.Dr. Michael ChernewDr. Michael Chernew is the Leonard D.Schaeffer Professor of Health Care Policy andthe Director of the Healthcare Markets andRegulation (HMR) Lab in the Departmentof Health Care Policy at Harvard MedicalSchool. Dr. Chernew’s research activities focuson several areas, most notably the causesand consequences of growth in health careexpenditures, payment reform, and Value BasedInsurance Design (VBID). Dr. Chernew is amember of the Congressional Budget Office’sPanel of Health Advisors and of the Instituteof Medicine Committee on National Statistics.Dr. Chernew is the former Vice Chair of theMedicare Payment Advisory Commission,which is an independent agency establishedto advise the U.S. Congress on issues affectingthe Medicare program. In 2000, 2004 and2011, he served on technical advisory panelsfor the Center for Medicare and MedicaidServices that reviewed the assumptions used bythe Medicare actuaries to assess the financialstatus of the Medicare trust funds. In April2015, Massachusetts Governor Charlie Bakerappointed Dr. Chernew to the MassachusettsHealth Connector Board of Directors. Dr.Chernew is a Research Associate of the NationalBureau of Economic Research. He currentlyserves as Co-Editor of the American Journalof Managed Care and Editor for the Journalof Health Economics. In 2010, Dr. Chernewwas elected to the Institute of Medicine ofthe National Academy of Sciences and servedon the Committee on the Determination ofEssential Health Benefits. Dr. Chernew earnedhis undergraduate degree from the Universityof Pennsylvania and a doctorate in economicsfrom Stanford University.

Dr. John Da SilvaDr. John D. Da Silva, Harvard School ofDental Medicine’s vice dean, was born inNew York City. He attended Williams Collegeas an undergraduate and received his DMDfrom the Harvard School of Dental Medicineand his MPH from the Harvard School ofPublic Health in 1987. Dr. Da Silva returnedto the School of Public Health and receivedhis ScM in health policy and management.He completed specialty training in implantdentistry and prosthodontics at HSDM in 1992.In 1998, he was appointed director of AdvancedGraduate Education. He served as chair ofthe Department of Restorative Dentistry andBiomaterials Sciences from 2010 to 2013. Dr.Da Silva has been actively involved in theHarvard Dental Center as the medical directorand HIPAA privacy officer. In addition to theseresponsibilities, Dr. Da Silva has made majorcontributions in research in the area of colorscience. He has also been involved in curricularchange to improve content on substance-abusescreening and brief interventions.for-profit ownership of insurance companies.Dr. Dafny graduated summa cum laude fromHarvard College and worked as a consultantwith McKinsey & Company prior to earningher PhD in economics from the MassachusettsInstitute of Technology. She has won severalteaching awards as well as Kellogg’s leadingresearch prize, the Stanley Reiter Best Paperaward. Dr. Dafny is a Research Associate ofthe National Bureau of Economic Research,Editor of The International Journal of HealthEconomics and Management, and a BoardMember of the American Society of HealthEconomists and the Health Care Cost Institute.Dr. Dafny’s expertise spans both the public andprivate sectors. She currently serves on the Panelof Health Advisers for the Congressional BudgetOffice. In 2012-2013, she was Deputy Directorfor Healthcare and Antitrust in the Bureau ofEconomics at the Federal Trade Commission inWashington, DC. Dr. Dafny advises companies,government agencies, and nonprofits on avariety of issues including antitrust matters,strategic decisions, and public policy.Dr. R. Bruce DonoffDr. Leemore DafnyDr. Leemore Dafny is the MBA Class of 1960Professor of Business Administration at theHarvard Business School. Prior to joiningthe Harvard faculty in 2016, she served asDirector of Health Enterprise Management andProfessor of Strategy at the Kellogg School ofManagement at Northwestern University. Herresearch examines competitive interactionsamong payers and providers of healthcareservices, and the intersection of industry andpublic policy. Dr. Dafny’s work has beenpublished in academic journals such as theAmerican Economic Review and the NewEngland Journal of Medicine, and featured inpopular media such as The New York Times andThe Wall Street Journal. Current projects includestudies of consolidation in the U.S. hospitalindustry and the kidney dialysis industry,products and pricing on the public healthinsurance exchanges, copayment coupons forprescription drugs, and the implications ofDr. R. Bruce Donoff was born in New YorkCity. He attended Brooklyn College as anundergraduate, received his DMD from theHarvard School of Dental Medicine in 1967and his MD from the Harvard Medical Schoolin 1973. Dr. Donoff ’s professional career hascentered on Harvard’s Faculty of Medicineand the Massachusetts General Hospital’sDepartment of Oral and Maxillofacial Surgery.He began as an intern in 1967, served asChairman and Chief of Service from 1982through 1993, and continues to see patientstoday. Dr. Donoff is the Dean of the HarvardSchool of Dental Medicine, a position to whichhe was named in 1991. In addition to theseresponsibilities, Dr. Donoff has made majorcontributions in research to the specialty oforal and maxillofacial surgery with interestsin wound healing, bone graft survival, sensorynerve repair and oral cancer. He has publishedover one hundred papers, authored textbooks,and lectured worldwide. He recently helpedlaunch a new Harvard School of DentalMedicine Initiative - Integrating Oral Healthand Medicine, a project of great importanceto him. Dr. Donoff served twelve years on theBoard of the Oral and Maxillofacial SurgeryFoundation and is former President of theFriends of the National Institute of Dentaland Craniofacial Research. He is the editor ofthe MGH Manual of Oral and MaxillofacialSurgery and a member of the Editorial Boardof the Journal of Oral and Maxillofacial Surgeryand the Massachusetts Dental Society Journal.Dr. Donoff has received numerous honorsduring his academic career, including theAmerican Association of Oral and MaxillofacialSurgeons Research Recognition Award, theWilliam J. Gies Foundation Award for Oral andMaxillofacial Surgery, Fellow of the AmericanAssociation for the Advancement of Science,the Alpha Omega Achievement Award andthe Distinguished Alumni and Faculty Awardsfrom the Harvard School of Dental Medicine.In 2014 he was a Shils-Meskin awardee forleadership in the dental profession.Michael J. DowlingMr. Michael J. Dowling is president andCEO of Northwell Health, which deliversworld-class clinical care throughout theNew York metropolitan area. Northwell isthe largest integrated healthcare system inNew York State with a workforce of about61,000 employees – the state’s largest privateemployer. With 21 hospitals and more than500 outpatient physician practices, Northwellis one of the nation’s largest health systems,with 9.5 billion in annual revenue. Prior tobecoming president and CEO in 2002, Mr.Dowling was the health system’s executive vicepresident and chief operating officer. Beforejoining Northwell Health in 1995, he was asenior vice president at Empire Blue Cross/Blue Shield. Mr. Dowling served in New YorkState government for 12 years, including sevenyears as state director of Health, Educationand Human Services and deputy secretaryto the governor. He was also commissionerof the New York State Department of SocialServices. Before his public service career, hewas a professor of social policy and assistantdean at the Fordham University GraduateSchool of Social Services, and director of theFordham campus in Westchester County. Mr.Dowling has been honored with many awardsover the years, including the 2011 Gail L.Warden Leadership Excellence Award from theNational Center for Healthcare Leadership andthe Ellis Island Medal of Honor. Mr. Dowling ischair of the Healthcare Institute and a memberof the North American Board of the SmurfitSchool of Business at University College,Dublin, Ireland, and the Institute for HealthcareImprovement (IHI). He also was an instructorat the Center for Continuing ProfessionalEducation at the Harvard T.H. Chan Schoolof Public Health. Mr. Dowling grew up inLimerick, Ireland. He earned his undergraduatedegree from University College Cork (UCC),Ireland, and his master’s degree from FordhamUniversity. He also has honorary doctoratesfrom University College Dublin, and HofstraUniversity.Dr. David S. GeskoDr. David S. Gesko is the Dental Director andSenior Vice President for HealthPartners.HealthPartners is an integrated, not-for-profit,consumer-governed health system based inMinnesota providing both plan (insurance)services along with the entire spectrum ofcare delivery including medical and dentalcare. Presently, HealthPartners serves over1.5 million members. Dr. Gesko is passionateabout integrating medical and dental care andapplying growing evidence-based research tocare delivery. “There is significant evidencelinking oral health and overall health. As anintegrated organization, HealthPartners holdsa key role in exploring this relationship andmanaging the members and patients we serveto deliver optimal overall health.” Dr. Geskowas appointed for a four year term by GovernorMark Dayton in 2012 as a board member forthe Minnesota Board of Dentistry. During histenure on the Board, Dr. Gesko has held all theofficer roles including President.

David L. HolmbergMr. David L. Holmberg is president and chiefexecutive officer (CEO) of Highmark Health, anational health and wellness enterprise basedin Pittsburgh, Pa., that serves more than 50million people nationwide. Passionate aboutsolving our nation’s most pressing healthcare problems, Mr. Holmberg is committedto improving the total health care experiencefor members, patients and consumers in arapidly changing health care landscape. Mr.Holmberg also serves as chairman of the boardof Highmark Inc., the health insurance affiliateof Highmark Health, as well as the Boardof Directors of the Blue Cross Blue ShieldAssociation and American Health InsurancePlans. Mr. Holmberg joined Highmark Inc. in2007, serving in a series of executive positions.Most recently he was president, DiversifiedBusinesses for Highmark Inc., where he wasresponsible for businesses representing morethan 3.5 billion in revenue. He is a Distinguished Alumni ofthe University of Texas at Dallas School ofManagement.Dr. Jan KimpenDr. Jan Kimpen is the Chief Medical Officerof Royal Philips, he has taken this positionas of January 2016. He joined the UniversityMedical Center Utrecht, one of the largesthealthcare organizations in the Netherlands, in1997 as professor and chairman of Pediatrics,and became President of the Board in 2009. AtUMC Utrecht, Dr. Kimpen strengthened tieswith the world-renowned Netherlands CancerInstitute at the Antoni van LeeuwenhoekHospital (NKI-AVL) and the NetherlandsPediatric Oncology Center. He has been thedriving force behind partnerships with Philipsand Eindhoven University of Technology,where he is a member of the strategic advisoryboard Health. Royal Philips has decided to addthis function to the Executive Managementteam since Royal Philips and the Healthcareindustry will undergo a huge transformation.Dr. Kimpen will support and build on RoyalPhilips integrated approach to coordinated careacross the Health continuum where Philips willfocus on delivering better patient outcomesat lower costs. LenoxMs. Erin T. Lenox is a senior consultant inthe Health and Benefits practice of WillisTowers Watson and specializes in the strategy,design and delivery of employer-sponsoredgroup health and welfare plans. Ms. Lenoxhas extensive experience consulting with largecorporations and designing comprehensivehealth risk management strategies to addressthe complex components of the US healthcaredelivery system. Ms. Lenox works with clientsto develop targeted, data driven solutions thatestablish and maintain high performing andcost-effective employee benefit programs. Acarefully designed configuration of vendorpartners, plan designs, funding methodologies,communications and employer subsidizationis central to optimizing health outcomesand maximizing the engagement of both theemployee and his or her family members.Ms. Lenox also facilitates the integration ofdata across the diverse set of vendor partnersand external stakeholders that support anemployer’s group health plan. Her research has examined theeffects of policies designed to increase healthinsurance coverage or improve the quality ofcoverage on access to care and health outcomeswith a focus on vulnerable populations.
