DEPARTMENT OF PHARMACYThe Pharmacy Department services both Wolfson Children’s Hospital and Baptist Medical Center. The Pharmacy provides24-hour inpatient support for both facilities. The department is staffed with approximately 99 employees includingtechnicians, centralized and decentralized pharmacists, pharmacy residents and interns, administrative personnel, andclerical support.The department utilizes automated drug delivery units and profile dispense throughout the hospital. A large percentageof our unit dose medications are packaged onsite.Pediatric and Adult decentralized clinical pharmacists provide clinical pharmacy services to their respective area ofpractice. WOLFSON CHILDREN’S HOSPITAL/BAPTIST HEALTHRESIDENCY PROGRAMWolfson Children’s Hospital/Baptist Health PGY1 Pharmacy Residency Program is a one-year training program beginningnear the first of July and ending the next year at the end of June.Purpose: Wolfson Children’s Hospital/Baptist Health PGY1 Pharmacy residency program builds on Doctor of Pharmacy(Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medicationrelated care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate yeartwo (PGY2) pharmacy residency training.Wolfson Children’s Hospital/Baptist Health offers residents the opportunity to complete one of two tracks, PGY1 at BaptistMedical Center-Jacksonville (adult hospital) or PGY1 at Wolfson children’s Hospital. The program of each resident variesbased upon his or her goals, interests, and experience. Each resident is required to complete core rotations essentialto the pharmacy practitioner. PGY1REQUIRED ROTATIONS – PGY1BAPTIST MEDICAL CENTER JACKSONVILLEOrientation (start date to end of July)The resident will be orientation to the purpose and practiceenvironment, the PGY1 standards, the PGY1 competencies,goals and objectives, the design of the residency programincluding all program requirements, description of requiredand elective learning experiences, evaluation strategy,resident manual, resident policies, terms and conditions,requirements of completion and education on burnout, risksand mitigation strategies.Internal MedicineThe resident will participate in daily patient care activitiesincluding collaborating with Physicians, Nurse Practitioners,Physicians Assistants, Nurses, Nurse Managers and SocialWorkers. The goals for all activities are to optimize drugtherapy, deliver high quality pharmacy services to patientsincluding drug and disease state counseling and function as amember of the health care team. Furthermore, residents will beable to monitor efficacy and toxicity of medications and identifyand manage adverse drug reactions thereby contributing toincreased patient satisfaction and quality of life.Adult CardiologyThe resident will gain experience in thepharmacotherapeutic care required by cardiology patients.The resident will participate in daily patient care monitoringand a variety of interdisciplinary activities. The residentwill develop an understanding of the management ofhypertension, heart failure, cardiac arrhythmias, acutemyocardial infarctions, interventional cardiac care andsurgical cardiac care.PsychiatryThe resident will participate in daily rounds with thepsychiatrist and other disciplines as needed. The residentwill offer pharmaceutical care for both psychiatric andmedical issues. Additionally, the resident will initiate patientinterviews to discuss the patient’s treatment regimen andmonitor the patient using several verified scales. The mainareas of focus are depression, bipolar, schizophrenia,anxiety, and personality disorders.Drug InformationThe resident will gain experience in providing druginformation to health care professionals and patients.The resident will be involved in various activities includingliterature retrieval and evaluation, formulary management,policy development, investigational drug services, andwriting and editing drug information publications. Theresident will participate in Pharmacy and TherapeuticsCommittee activities including medication use evaluation,monograph development, and physician education.Pediatric ExperienceThe resident is required to take one of the pediatricexperiences listed in the PGY1-Wolfson Children’s Hospitaltrack that will provide exposure to a variety of disease states(general pediatrics, pediatric hematology/oncology,pediatric emergency department).Research (one month-December)The resident will have one month to collect data and makeprogress on their research project. During this month theresident is expected to meet with the research associates toreceive feedback on data collection and make changes inthe process if needed.One of the following:Adult Critical Care (Medical/Surgical)The resident will gain experience in renderingpharmaceutical care to critically ill patients. The residentshall become proficient in recognizing the appropriate drugtherapy for this patient population. The resident willbecome familiar with common medical emergencies andcritical illnesses. Also, the resident will be exposed to postoperative care of surgical patients. The resident will meetwith multidisciplinary members of the critical care teamdaily to discuss patient care.Adult Neurology (Critical Care)This rotation will provide the resident with drug therapymanagement of hemorrhagic and ischemic stroke patientsin a primary certified stroke center. The resident willbe required to follow the patient from the EmergencyDepartment throughout hospitalization until discharge torehabilitation or long term care facility. The resident willmonitor efficacy and toxicity of medications and identifyand manage adverse drug reactions. PGY1ELECTIVE ROTATIONS – PGY1BAPTIST MEDICAL CENTER JACKSONVILLEAdult Emergency MedicineClinical Toxicology/Poison InformationThis rotation will provide the resident with a unique andchallenging experience, structured to combine didacticknowledge with practical applications within the emergencydepartment. Pharmacy practice in this environment willrequire rapid, accurate retrieval of information. Moreimportantly, the communication skills of the resident will beessential, as physician, nursing and patient interactions arevital. Residents will be exposed to a broad range of topicsincluding but not limited to critical care, internal medicine,neurology, toxicology and psychiatry.The rotation will focus on and familiarize the resident withpharmacy practice in the Emergency Department and PoisonInformation Center. Urgent and emergent therapeutics,clinical toxicology, and the provision of poison informationwill be emphasized allowing the resident to discusspathophysiologic entities and formulate therapeutic andmonitoring protocols for patients on a case-by-case basis.Adult OncologyThe resident will participate in daily management ofdrug therapy and pharmaceutical care of adult oncologypatients. The resident will become familiar with commonchemotherapy regimens and supportive care. Areas to becovered include pain management, antiemetic therapy,infectious diseases, and nutritional support. Inpatient andoutpatient experiences are available.Infectious DiseaseThe resident will gain experience and proficiency in theclinical care required by the complex nature of bacterialand viral infections observed in the hospital setting. Theresident will learn and implement AntimicrobialStewardshipprincipals. The resident will participate in dailypatient care rounds and a variety of interdisciplinaryactivities. The resident will develop an understanding ofinfectiousorganisms, antibiotic combinations andpharmacokinetics.Investigational Drug ServiceMedication SafetyThe experience focuses on the development of clinical andleadership skills in healthcare. The resident will gainexperience with the implementation and management ofNational Patient Safety Goals, ADE surveillance,Computerized Prescriber Order Entry (CPOE), automateddispensing cabinets, barcoding, clinical documentation,application of build rules/care sets. The rotation willprovide opportunities for integrating the pharmaceuticalcare practice model in a hospital setting.Additional Elective RotationsResidents in the Baptist Medical Center-Jacksonville trackmay also take any pediatric rotation listed in the WolfsonChildren’s track, depending on availability. Residents havethe opportunity to repeat a learning experience once duringthe residency year. They may also take the critical carerotation not chosen to fulfill the critical care requirement.Electives may be chosen to fulfill the residents goals, butno more than 1/3 of the residency may in one patientpopulation. Other electives may be available at other sites inthe city as well as new electives may be developed.This rotation will provide residents with a workingknowledge of investigational drug management andfamiliarize them with basic principles of clinical research.Practical experience in investigational drug managementwill be provided. The residentwill offer pharmaceutical care for both psychiatric andmedical issues. Additionally, the resident will initiate patientinterviews to discuss the patient’s treatment regimen andmonitor the patient using several verified scales. The resident will become familiarwith common pediatric emergencies and critical illnesses.Also, the resident will be exposed to post-operative careof cardiothoracic patients. Introduction to the commoncongenital heart lesions and their pharmacological oroperative management is an integral part of this rotationalexperience.Neonatal Intensive CareThe resident will gain experience in caring for the uniqueneeds of critical neonates on both a clinical and practicalbasis. This will be accomplished by gaining proficiency inneonatal pharmacotherapy and dealing with the day to daychallenges of providing care to a variety of patients includingvery low birth weight neonates and infants with variouscongenital malformations, such as genetic disorders, neuraltube defects and congenital heart defects. A significant goalof this rotation will be development of an integral knowledgeof all technology and equipment used in the neonatalpopulation that may require special skills and/or knowledgeto safely and effectively provide medications to neonatesand infants of various ages.Drug InformationThe resident will gain experience in providing druginformation to health care professionals and patients.The resident will be involved in various activities includingliterature retrieval and evaluation, formulary management,policy development, investigational drug services, andwriting and editing drug information publications. Activities and involvement will be basedupon the control and regulation of investigational drugsthroughout the institution.Pediatric NutritionThe purpose of the pediatric nutrition rotation is to providean educational and training experience for a pharmacistin the fundamentals of pediatric nutrition therapy andmanagement. Emphasis will be placed on developingthe resident’s communication skills in the field of nutritionsupport. The resident will participate in daily patient caremonitoring and a variety of interdisciplinary activities. Thisrotation is designed to provide the resident with the basictools needed to understand, rationally discuss and continueself-study in pediatric nutrition support.Pediatric Infectious DiseaseThe resident will gain experience and proficiency in theclinical care required by the complex nature of bacterial andviral infections observed in pediatric patients. The residentwill participate in daily patient care rounds anda variety of interdisciplinary activities. The resident willdevelop an understanding of infectious organisms,antibiotic combinations and pharmacokinetics. Through thisrotation, the resident will increase the therapeuticknowledge and development of practice skills necessary tomake appropriate recommendations regarding drugtherapy and pharmaceutical interactions observed in HIV/AIDS and infectious disease patients.Pediatric Emergency MedicineThe Children’s Emergency Center is a 23 bed unit withan 8 bed Fast Track unit which serves pediatric patientsthroughout the region. On this rotation, provision of safeand effective patient care for all patients is the mostimportantresponsibility of the pharmacist and resident. The learningexperience will allow pharmacy residents to becomefamiliar with the role of the pharmacist in the pediatricemergency department. Routine responsibilities include:Independent daily rounds, medication reconciliation,pediatric dosing recommendations, antimicrobialstewardship, pain/sedation monitoring, medicationdischarge counseling, IV medication preparation,participation in pediatric resuscitations/codes, and assuringappropriate pharmacotherapy on all pediatric ED patients.The pharmacist provides drug information and education tohealthcare professionals as well as patients and caregivers.Clinical Toxicology/Poison InformationThe rotation will focus on and familiarize the resident withpharmacy practice in the Emergency Department andPoison Information Center. Urgent and emergenttherapeutics, clinical toxicology, and the provision ofpoison information will be emphasized allowing theresident to discuss pathophysiologic entities and formulatetherapeutic and monitoring protocols for patients on acase-by-case basis.Medication SafetyThe experience focuses on the development of clinical andleadership skills in healthcare. LONGITUDINAL EXPERIENCES - PGY1PGY1-BAPTIST MEDICAL CENTER JACKSONVILLEPGY1-WOLFSON CHILDREN’S HOSPITALAdministration/LeadershipResearchThe resident will be introduced to and receive experiences inall aspects of Pharmacy Administration including Budgeting,Human Resources issues, Committee responsibilities, interand intra departmental relationships, and extra-hospitalresponsibilities during this year long experience. As partof this experience, the resident will serve as chief resident.Responsibilities include chairing the every other residentmeeting, leading their co-residents in a group project relatedto the pharmacy or residency administration and arrangingfor one social event involving the pharmacy department.Each resident must complete a research project during thisyear-long experience. This learning experience will guide theresident through project development, proposal writing, IRBsubmission, data collection, data analysis and presentationof the project to the hospital and the residency conference.A final manuscript suitable for publication is required. Atimeline/guideline is provided to each resident. A mentorwill guide the resident throughout the year.EducationThe resident will be given opportunities to provideeducation to physicians, nurses, pharmacists, pharmacytechnicians and pharmacy students during this year longexperience. At a minimum each resident will provide onePharmacy Grand Rounds, one CE for pharmacy technicians,and three journal clubs presented to the pharmacydepartment and two Lunch & Learns. Ten drug informationquestions are also required. They will also assist withprecepting pharmacy students.ServiceThis year-long experience provides the resident experiencecovering the central pharmacy on the weekend anddeployed on the weekday evening. The resident will serviceas the pharmacist for their assigned area. Weekend coveragemay include the IV room as well as the distribution processesfor both the adult and children’s hospital. Evening coverageis deployed for several medical units. During the weekdayevening, the resident will cover units for the hospital oftheir corresponding track.
