New York State – Department of HealthBureau of Emergency Medical Serviceswww.nyhealth.gov/nysdoh/ems/mainMISSIONTo ensure that quality and competent emergency medical servicesare provided by the EMS agencies in the State of New York by:1) monitoring EMS service operations;2) providing guidance and education to service management and personnel;3) reviewing patient care practices of certified care providers; and4) taking appropriate corrective actions.BACKGROUNDEMS services in New York State have been submitting paper patient care reports (PCRs) to the NewYork State Department of Health’s Bureau of EMS (BEMS) per Public Health Law Article 30-A Section 3053since 1996. In 2009, BEMS utilized grant funding to develop state bridge software to receive these PCRselectronically (e-PCR) from over 1,200 certified EMS services and nearly 800 first response agencies. Inaddition, the funding supported the alignment of New York’s e-PCR data with the National EMS InformationSystem (NEMSIS) dataset so that New York’s data could be integrated at a national level.This NYS PCR Data Dictionary modifies the national NEMSIS data dictionary by specifying what dataelements, in addition to the NEMSIS minimum dataset, New York State requires for submission by itsEMS services. In addition to the 89 NEMSIS minimum dataset, NYS requires 126 data elements for a totalof 215 data elements to be collected for submission to the NYS e-PCR state bridge.Questions regarding submission of PCR and e-PCR data can be address to the Regional EMS ProgramAgencies found at: A Uniform Pre-Hospital EMS Dataset Version 2.2.1National EMS Information Agency Project (NEMSIS)www.nemsis.orgIn 1994, Version 1 of the U.S. National Highway Traffic Safety Administration’s (NHTSA) Uniform PreHospital EMS Dataset was created through a consensus based national data conference. The NHTSAVersion 1 EMS Dataset has been acknowledged as the national standard for the EMS industry. Subsequentupdates and revisions (also accomplished through a naional concensus process) have created the currentVersion 2.2.1 Dataset.The NHTSA EMS Uniform PreHospital Dataset, Version 2.2.1 is composed of three separatecomponents. A demographic dataset ("D" elements) provides a standardized set of definitions describing anEMS System/Agency; while an EMS event dataset ("E" elements) provides a standardized set of definitionsdescribing an EMS event. The third component of the Version 2.2.1 standard is an XML file format anddefinition created to promote the movement of the Version 2.2.1 data elements between data systems.Any implementation of the NHTSA/NEMSIS Version 2.2.1 dataset must include the use of thedemographic, EMS event, and XML dataset standards.NYS EMS PCR Data Dictionary (Issue 2011–1)Page 2 of 478

This NYS PCR Data Dictionary provides over 400 definitions which can be implemented by an EMSsystem. The NEMSIS initiative serves to provide technical assistance for the implementation of the dataset.National data elements are defined which should be collected by a National EMS Database, but additionaldata elements should be considered for use at the state and local levels depending on each state or localEMS system's need. The goal of NEMSIS is to establish an EMS data system at the local, state, andnational levels.
