NCFlex BenefitsThe University of North Carolina (UNC) System offers a flexible and comprehensive package ofbenefits provided through the N.C. State Health Plan, NCFlex state insurance plans, and otherUniversity-sponsored programs. These programs are designed to allow you to tailor a benefitspackage that best meets the needs of you and your family.This guide provides an overview of the plans available through NCFlex. You may enroll in any orall of the NCFlex benefits, if eligible. Yourbenefit elections are effective on thefirst day of the month following yourdate of hire.Open Enrollment is your once-a-yearopportunity to review and select yourbenefits for the coming year, add orcancel dependent coverage, and enrollin the Flexible Spending Accounts.You can enroll or make changes to yourbenefit elections during the year if youhave a qualifying life event (see listbelow). You must enroll/make changeswithin 30 days of the qualifying event.If you don’t enroll within 30 days, youwill not have any NCFlex benefits for theremainder of the plan year. Your nextchance to enroll will be next fall duringOpen Enrollment for the followingplan year, or when you experience aqualifying life event that would allowyou to add or drop a dependent—likegetting married or divorced.Open Enrollment occurs during the fall.Qualifying life events include, but arenot limited to:If you do not enroll during the OpenEnrollment period, your currentelections will roll over, except for anyflexible spending account elections(Health Care Flexible Spending Accountor Dependent Day Care FlexibleSpending Account), which must beelected each year. Marriage Divorce or legal separation Birth or adoption (or placement ofadoption or foster) of a child Death of a covered dependent Change in your spouse’s employment,impacting his/her benefits eligibility Your dependent turns age 26Any change you make in coverage mustbe consistent with your status change.3

How to EnrollYou can enroll in NCFlex benefits in one of two ways:Take ActionOnline through the BenefitsEnrollment Platform All institutions have single sign-on for onlineenrollment. Click here and select your institution.Enter your institution Login ID and Password. Select “Get Started” on the home page andfollow the prompts. After you have made your choices, and they aredisplayed for you to review and print, you MUSTscroll down to the bottom and click “Save Changes”or your choices will not be recorded! Don’toverlook this critical step! Print a copy of your Confirmation Statement beforelogging out.Call 855-859-0966Call the eligibility and enrollment call center at855‑859-0966, Monday – Friday, 8 a.m. – 5 p.m., ET.Benefit TipYour costs or contributions for NCFlex benefits are madeon a pre-tax basis. Pre-tax benefits let you pay for coveragewith dollars from your pay before taxes have beendeducted, which results in tax savings for you.4When you log in to the enrollment system,be sure to: Review your contact information (phone andemail) and be sure your mailing address is correct.If any of your current information is incorrect,you will need to update it within your institution’sHR/Payroll system. Enter or update your dependent information,including date of birth and Social Securitynumber, for each dependent you want to enroll. Review, add, and update beneficiaries on yourTerm Life, AD&D, Critical Illness, and Cancer plans,as needed. Print a confirmation statement after you haveelected your benefits so you’ll have a record ofyour choices.

EligibilityYouImportant Dependent CoverageRemindersYou may enroll in any of the NCFlex benefits if you are: A permanent (non-temporary) employee who works 30 ormore hours per week, or You must be enrolled in a plan for your eligibledependent(s) to participate. A permanent (non-temporary) part-time employee whoworks 20 to 29 hours per week. You may not be covered as both an employee and adependent and children may not be dually enrolled.If you have questions about your eligibility, contact yourUniversity Benefits Representative (UBR). You should consult with your tax advisor if you havequestions as to whether someone qualifies as yourincome tax dependent.Your Dependents Dependents do not have to be enrolled on yourhealth plan in order to be enrolled on your NCFlexplan(s).Your eligible dependents include: Your spouse (includes same gender marriage) Your children up to age 26, including natural, legally adopted,foster children, children for which you have legal guardianshipand your stepchildren For the TRICARE Supplement, eligibility is up to age 21, orup to age 23 if enrolled full-time in a school of higher learning. Your children of any age who are physically or mentallyincapacitated, to the extent that they are incapable of earninga living, and such handicap developed or began to developbefore the dependent’s 26th birthday while they were enrolledin the plan.Benefit TipIf you plan to enroll dependents, allow yourself enoughtime to gather any required documentation, such asdependents’ Social Security numbers and dates of birth.5

DentalGood oral health is an essential part of a healthy lifestyle. Through MetLife, NCFlex offers threedental plan options that cover routine checkups and other dental care: the High Option plan, theClassic Option plan, and the Low Option plan. These options differ both in how much you pay perpay period and at time of service.Covered ServicesMonthly Cost for CoverageNo matter which dental plan option you elect, you can visitany licensed dentist, in or out of the MetLife Preferred DentalProvider (PDP) Plus Network, and still receive benefits. Alldental plan options provide benefits for: Diagnostic and preventive services, such as oral exams,cleanings, and X-rays. Basic services, such as fillings, extractions, root canal therapy,and treatment of gum disease.Coverage LevelHighOptionClassicOptionLowOptionEmployee Only 44.56 33.08 21.08Employee and Spouse 89.36 66.32 42.50Employee and Child(ren) 96.36 71.88 45.64Employee and Family 157.78 112.98 72.74Which Plan Is Right for You?The Classic Option plan and High Option plan also cover:The Classic Option offers a higher level of benefits than theLow Option and a lower level of benefits than the High Option.The Classic Option may be a good option for you if you wantcoverage for Major services, such as crowns and dentures, ororthodontia services, but you don’t need the highest level ofcoverage that the High Option offers (and do not want to paythe higher premium for coverage). Major services, such as crowns, dentures, and bridges. Orthodontic services for dependent children up to age 19,with a lifetime maximum benefit of 1,500.Save When You Use a NetworkProviderThe High Option may be right for you if you need the highestlevel of coverage for basic and major services and a higherannual maximum, and you are comfortable paying thehigher premium for coverage. See the “Dental At a Glance”comparison chart on the next page for details.Even though you can see any licensed dentist for care, you cansave money when you visit a MetLife Preferred Dental Provider(PDP). That’s because providers in the MetLife PDP networkcharge negotiated rates that are typically 30-45% less than theaverage charge in the same community. To find a participatingdentist, go to metlife.com/mybenefits, enter “NCFlex” as thecompany name and enter your ZIP code. You can also call855‑676-9441 to request that a provider list be sent to you.Benefit TipYou can also use your Health Care Flexible SpendingAccount to pay for eligible dental expenses (that are notcovered by the dental plan) on a pre-tax basis. Keep in mindthat cosmetic procedures, such as teeth whitening, are notconsidered eligible expenses.6

Her job at the University of North Carolina System Office is her first“real” job. She’s heard that the NCFlex benefits are great but she’s a little overwhelmedcomparing her options. What’s best for Jen? Well, she’s never had a cavity in her life, so thedental plan option is an easy one—she’ll go with the Low Option so she can get her routinecleanings and have coverage for basic services in case something comes up. And visioncoverage is a must because she wears glasses—she chooses the Basic coverage because shedoesn’t want to pay the higher premium for Enhanced coverage. Jen decides to pass on theother NCFlex benefits for now.7

VisionNCFlex offers vision coverage through EyeMed Vision Care to save money on eye exams andeyewear. You can see any vision provider you choose. The level of benefits you receive dependson whether you go in-network or out-of-network for services.The Vision Plan offers three options: Core, Basic, and Enhanced. Core vision coverage is availableto you at no cost, if you enroll, and covers an annual eye exam with a 20 copay. Both the Basicand Enhanced options provide a comprehensive eye exam and benefits for vision materials. Youmay receive either eyeglass lenses or contact lenses in a benefit period but not both.Monthly Cost for CoverageBenefit TipYour monthly vision premium is based on the option youchoose and whether you elect to cover yourself only, oryourself and your family.
