Section I - IntroductionThis directory provides a list of HealthPartners Freedom’s network providers. To get detailedinformation about your health care coverage, pleasesee your Evidence of Coverage (EOC).You generally must receive your care from a networkprovider for our plan to cover the services. However,you can choose to use out-of-network providers.“Non-plan providers” aren’t part of the Freedomnetwork. If you use out-of-network providers for carethat isn’t emergency care, including maintenanceand hospital care after you’re stable (known as poststabilization care), urgently needed care, or haven’tstarted your Extended Absence benefit, you will paythe cost-sharing amounts under Original Medicare.Upon activation, your Extended Absence benefitprovides the same level of coverage when you’reoutside the service area and within the United States.The network providers listed in this directory haveagreed to provide you with your health care, visioncare and dental services. You may go to any of ournetwork providers listed in this directory; however,some services may require prior authorization. Otherproviders are available in the network.2We cover certain health care services that you getfrom out-of-network providers. These include care forthe following: medical emergency, including hospital care afteryou’re stable maintenance care urgently needed care when you’re temporarily outof the service area care that has been approved in advance byHealthPartners services which were determined to be coveredthrough an appeals process covered under the Extended Absence benefit.If an out-of-network provider asks you to pay forcovered services, please contact Member Servicesat the numbers on the inside front cover. Box 1289Minneapolis, MN 55440-1289When you are outside the service area and cannotget care from a network provider, you may receiveemergency and urgently needed care from the closestavailable provider. In addition, when out of theservice area you can obtain dialysis treatment fromany qualified dialysis provider. With the exception ofemergencies or urgent care, it may cost more to getcare from out-of-network providers.
